Maintenance Request Form

The form below is for your convenience in requesting any repair/maintenance. You must submit a service request in writing in order for SDL to dispatch any service company to your residence. Please keep in mind that there are certain repairs that are the tenants responsibility; clogged drains, and toilets, beeping smoke alarms( battery needs replacing), light bulbs burning out. We will make every attempt to service any request that the owner is responsible for in a reasonable amount of time. If a non emergency service request is placed on a Friday afternoon, we will contact you the following Monday. Emergencies are considered water leaks that cannot be stopped by turning the shut off valve yourself, or anything that can cause an immediate fire or hazzard. If an emergency situation occurs during business hours call 951-302-5300. AFTER hours Emergency # 951-764-8486 IF YOU HAVE A FIRE OR GAS LEAK PLEASE CALL 9-1-1 !!!!
