Your rental/lease agreement requires you to give at least a 30 day written notice to vacate. You may use this form to fullfil this obligation. The form can be used for all notices to vacate, but be advised that if you are not in compliance with your lease agreement and are vacating before the term of your lease, you will be held responsible for all terms stated in your lease regarding the early termination of your lease.
